SED Waiver

Keeping Families Together Through Coordinated Care

The Serious Emotional Disturbance (SED) Waiver helps youth with severe behavioral health challenges stay in their homes and communities instead of being hospitalized.

PACES and Wyandot Behavioral Health Network are providers under this Medicaid waiver program. We help connect children and teens with services that meet their needs and empower families with the tools and services that can create more stable, supportive home environments.

Getting Started With the SED Waiver and Services

Once your child qualifies for the SED waiver, a coordinator will create a care plan within 14 days and schedule a planning meeting within 30 days to finalize services and support.

Referral & Intake

Contact PACES or your child’s care provider to begin the referral process and explore their eligibility.

Assessment & Approval

A clinical team will conduct evaluations to determine your child’s needs and readiness for waiver services.

Services Begin

Once approved, you and your child will be assigned a Wraparound Facilitator who will coordinate your care and services.
