Delivered through our team at Kim Wilson Housing, ACT meets individuals where they are—at home, work, or in their community—to provide intensive, ongoing mental and behavioral health care. This service is designed for people who need more than traditional outpatient care to maintain stability.

Referral or Inquiry
Individuals are often referred by providers, but can also reach out directly to be screened for eligibility by our case management team.
