Individuals can be referred by an internal provider or community partner or contact WBHN to begin the care coordination process.

We conduct an assessment to identify your needs and determine the most appropriate services and supports.
Ongoing Coordination
Our team stays involved to monitor progress, adjust resources as needs change, and ensure clear communication across your care and support network.

Yes. We offer care coordination services in English and Spanish, and can provide interpretation so every individual and family receives support in the language they feel most comfortable using.
The frequency of your meetings is based on your goals and circumstances. You will likely have a greater level of communication when you first reach out seeking services to help get you connected to the supports you need. Your care coordinator will check in regularly and remain available when new needs arise or additional support is requested.
